How to begin the online Intake Process

If you would like to request help, you can begin the process online anytime or in-person during regular office hours.  If you need help completing the form, a member of our team can help you; just come to one of our office locations during regular hours.

Once the online intake form has been received, a member of our team will contact you to schedule an in-person interview.  You will need to bring a photo ID, as well as any other requested documentation, with you to your appointment.  

Please note the following: Forms will be collected on the next business day.  You must come in person to meet with our team.  We do not provide assistance online or over the phone.
